Insurance Plans

The following are insurances with which we contract and/or will submit payment requests on our patients' behalf.  This list is subject to change without notice or update on this website.  Please contact our administration for the most up to date information or to confirm if we are currently contracted with your insurance company.

Seeing a provider Out of Network

We routinely see patients out of network, meaning you are paying cash upfront, just as you would a hairdresser, a massage therapist or a lawyer.  In some instances, patients have insurance coverage, but it's with a company with which we don't have a contract.  If so, you may be able to submit your bill to your insurance company and receive some refund towards your payment made.

Important: Veterans Affairs Insurance


Effective 12/12/2025

After careful consideration, our practice will no longer accept Veterans Affairs (VA) insurance for new or renewed authorizations. This difficult decision comes after ongoing administrative challenges that prevent us from providing the timely, quality care our patients deserve.

What This Means for You

If you currently have an active Request for Service (RFS) on file:

  • We will honor your existing RFS through its expiration date
  • RFS authorizations will not be renewed after expiration
  • You are responsible for tracking your RFS expiration date

If you do not have an active RFS:

  • You may use your Medicare benefits at our practice
  • You will be responsible for the standard 20% coinsurance unless you have supplemental coverage

If you have a pending RFS for an upcoming appointment:

  • You may reschedule to a later date once your RFS is approved, OR
  • You may keep your appointment and use your Medicare benefits

Our Commitment to Veterans

This decision does not reflect our appreciation for your service. As a practice administrator with a family legacy of military service—including a father who served 20 years in the Army and earned three Purple Hearts—I understand the sacrifices veterans have made for our country. Additionally, three of Dr. Carlsen's grandparents served in the military, with his paternal grandparents meeting during their deployment in the Korean War. Neither of us took this decision lightly.

However, systemic delays in VA authorization processes, denials based on technical date discrepancies, and non-payment for services rendered have made it impossible for our small practice to continue participating in the VA network while maintaining the standard of care you deserve. During the most recent government shutdown, we experienced significant payment delays that further strained our ability to meet payroll and operational expenses. In other words, this has nothing to do with our desire to care for our veterans and everything to do with the VA delaying care and refusing to reimburse for services provided.

On an island with a critical shortage of healthcare providers and wait times already extending weeks to months, we cannot sustain the additional administrative burden that diverts resources from patient care.
